Supply List and Other Forms

2018-2019 Information

Families who have enrolled for the 2018-2019 school year will receive a packet in July containing a supply list and a few forms that need to be filled out and returned to the school on or before Orientation.

• 2019-2020 Photo Authorizations
• 2019-2020 Approved Drivers List
• Immunizations Form (Note: Parents must fill out form for New Students; for Returning Students, Parents can update the old form at Orientation)
